I agree with your point on the negative media attention on hold-outs. I think that is changing somewhat. I think people made a big deal out of it with TO because he was already a superstar before going to Philadelphia and his deal was only a year old.
I think TO was the most visible situation. But I think that people generally think that players are overpaid spoiled brats (which they are) and ownership generally isn't because ownership is somehow different or because presumably they made the money to buy the team outside of the sport and I think it's a bullshit distinction.

I would speculate that potential insurers would demand a lot to bear that risk, leading players to self-insure.

As I said before, maybe most players have convinced themselves that they are Elect, not Preterite, and certainly not the Damned (who never made it to the pros at all), and that if they act right and don't step on a crack, they'll avoid injury.
