Everyone definitely has to be appearing to enjoy themselves. Those who like giving blow jobs may be more willing to interpret the giver's expressions etc. as enjoyment than those who don't like giving blow jobs, who may interpret it as faked, or discomfort, or whatever. I mean, the giver can't really talk and the facial expressions are limited, so there's a lot of room for interpretation.
I would interpret gagging noises as a bad thing, though. Would not want to watch blow jobs that involve the giver gagging.
I actually have rented PPV at a hotel that was bj-centric. And reordered it the next night.
