I think that in many cases it does attack people rather quickly. Not overnight, but many in under a year. I also think that smokers and drinkers tend to realize their illnesses later in the game -- like smokers are used to having a cough and don't go to a doctor until they cough up half a cancerous lung one day. Most smokers and drinkers I know just don't go to the doctor very often. On the other hand, the fatties I know seem to be otherwise very concerned about their health and go to the doctor for every ache and pain (but would never admit that the extra tonnage may be causing the problems). Not saying that smoker and drinkers are tougher or something but I think they, in general, don't seem to give as much of a shit.
