Coltrane is smarter than you give him credit for- he means "clearly need" on a societal level - childbirth, earth mother, etc.
Meanwhile societies like the amazons got alone quite well by killing off all the male children except the most physically fit- Coltrane knows he would have been put to the sword instantly, Ironhead, Flower all them- BECAUSE the amazons would have a Hank Chinaski-like breeder around---- nickname--- LEVIATHAN!
