Another important element is that the guys in question WANT to be "improved," and as you know, many lesbians look the way they do not because they don't know how to dress better, but because they are explicitly rejecting larger societal notions of feminine attractiveness. They view their appearance (or lack of attention to such matters) as a sort of political statement.
