Skip unless you really care about HIPAA.

A covered entity is a) a health plan, b) a health care clearinghouse, or c) a health care provider that who transmits any health information in electronic form in connection with a transaction [covered by the administrative simplification parts of HIPAA].

If they don't submit electronic claims to health insurers, then they're not covered entities under HIPAA. Lots of health care providers are either subcontractors or bill directly to the patient or otherwise not involved with electronic claims submission.

'Course, they're probably still covered under California's medical privacy protection laws. But not HIPAA.
