As a former crack Pony Club member, I am well positioned to answer your question.

A crank noseband differs from a normal noseband because it is not just one one piece of leather making up the noseband. It has a front piece that is joined at the back by a separate strap. If I remember correctly, it is supposed to be to apply even pressure all around the horse's nose. It is for better control. It is wider and more heavily padded. Used in dressage more than just on a regular basis.
