A fetish is when something that wouldn't normally turn someone on becomes an object of gratification. Like feet do it for some people. This guy just liked the look of blood. It didn't mean anyone was actually bleeding or dead or that he was pretending that someone was. Hard to understand, but that's what it is. So not violent, but still weird and creepy.
