The prior post simply means that once upon a time you could watch the NYC locals from LA, or anywhere in the country. "Hey, 'Friends' is on in New York." Not anymore. Because DTV worked out deals with locals, they suck up local signals and shoot them back down through the dish in
many, but not all, localities, including (of course) NYC and LA. If you don't live in one of the places where locals are offered, you must either (1) pick them up OTA on a regular antenna (which means you can't Tivo them with a DirecTivo; long story); (2) get yourself basic-basic cable for locals only (ditto); (3) convince DTV that you're situated in a place where you can't get reception, in which case they'll send you the NBC, ABC, CBS etc. affiliates from your nearest megalopolis (which means you don't get the local news, no big loss).
