I think it comes from a "you have submitted yourself to me bodily, and I will (in the course of offering "medical care") touch you more intimately than you might want your own wife to, and I am a professional and this isn't personal so it is OK, and I am wearing rubber gloves while I do it" thing. And perhaps sort of a "mommy takes care of you thing" - it always struck me as a remarkably infantilizing fantasy. Not everyone's cup of tea, but I could see where some of this might appeal to some folk. The fact that this is a pretty heavily retro fantasy/fetish is telling - I think nurses were sort of the mid-century version of "co-eds": independent, "liberated" unmarried girls who were really all out there looking for a man.
That and, of course, the certain knowledge that all nurses are hot sluts who are banging all the doctors in the supply closets, as everyone knows.
