A) If I were designing restrooms, there would be larger dividers. But, I suspect every sq ft of restroom is another sq ft of cost.
B) This is really a corollary of the general "maximize personal space" rule - the same one that mandates how people stand in an elevator so as to approximately maximize the ring of personal space surrounding each person. As people get on or off the elevator at various floors, those on the elevator shift postions to account for the lost or gained space.
