"casualties" does not mean "deaths"
I'd blame it on inadequate concern for the citizenry in a communist dictatorship.

Neither casualties nor deaths have been confirmed by anyone who is willing to give a name. And it may be one of those things we never really learn, though the scale has to be huge for South Korea to be announcing their suspicions on casualties when it happened on the other side of N. Korea from them.
As one who believes things have reasons, and big things have big reasons, I think its safe to say that my theories are correct. Either its a.) Kim's hand to silence voices who know something or b.) China's hand sending a message to Kim, or c.) another nation that does not like Kim, trying to make it appear like a.), b.) or an accident (for those innocent enough to believe in such things).
Note, a.) is not diametrically opposed to your inadequate concern statement. For that matter, neither is b.) or c.) (i.e., it was pretty uncaring of him to get his country into this mess).
Hell(if you were Kim and you weren't going to nuke some part of America, what part would it be?)o
