I'm no expert on hepititis, but I think it must pass by blood to blood contact because Pam made a big point of saying she got it from sharing tattoo needles with Tommy (I'm not making this shit up). Thus it is not impossible to get during sex, but not as transmittable as most STDs. I assume that if either Angelina or Billy Bob had it, the other one would have caught it too.
Maybe Heather didn't share any tattoo needles with Tommy. Or he contracted it after his time with her was over...
