I think I've figured out this cover. She is halfway in the trunk - I don't know why, but presume the novel will explain -- and he has stopped giving her oral pleasure because he was suddenly gripped by severe intestinal pain -- which, along with the rash on his arm, we presume will be explained in the novel. As having been caused by a food allergy. To a food prepared for him by trunk lady.
