No, I'm serious. Just going around the outsides of the studs (like you could if you had e.g. two columns of nails in a board) wouldn't seem to work well in the context of mobile flesh, and if you did a complete loop around each stud before taking the ribbon/string to the next stud, the ribbon or string or whatever wouldn't be movable.
I don't see why this would hurt unless someone were trying to pull the labia apart while they were laced up.
