I guess technically, the ones I have seen are not documentaries as much as investigative news pieces. One was done by Diane Sawyer. ABC followed one girl, her stage name is Belladonna, around for two years and they did several interviews of other current and ex-porn stars. Those women were the more well known stars, not the anonymous eastern block girl.
I cannot remember where I saw the other news piece, but I think it may have been A&E. That one included this ex-porn star who now runs a social service program for current and ex-adult entertainment workers. They provide help with drug addiction and STD testing and refer for mental health services.
I just did a quick google search for pornography and documentary and found two documentaries.
One is a British documentary called HardCore that follows this 25 year old (Felicity) around as she travels from the UK to the US to enter the porn industry. Max Hardcore is in that one. I haven't seen it, just read the reviews online. It sounds very similar to the investigative news pieces.
Then there is another one called Rated X that you can read about here:

But even those news pieces and documentaries only show you the more mainstream hardcore industry. I have never seen one that shows what some of these eastern european hardcore porn shops are like. I have only read articles about them after a woman has been able to escape the life or turns up dead. Given that some of the women reportedly are underage or held against their will, I doubt a filmmaker can get access to film that segment of the industry. Czechoslovakia is supposed to be the worst for that in Europe.
