I think in the anon's example, it would be pretty easy to determine if he was using the term to denigrate his target because they are gay or if he was using the term because people who piss him off should generally be considered cocksuckers. It's one of those, "you had to be there/know it when you see it" standards.
Maybe the anon could clear it up.
