I'm thinking there would be a basic package of care that everyone would get, the particulars of which would be the same. People would then buy care above and beyond that with their own money, etc. So the companies competing to provide the basic level of care would all be providing the same thing, and would have to compete on price in bidding for that business. The government's role would be to define the level of the care in the basic package, to put the care out to bid, and to ensure that consumers get what was paid for.

The discussion of a two-tier health system has been going on in Canada for some time now.

Yes. Why tie it to drivers' licenses? Why not issue licenses to breath--call them green cards--and make buying health insurance a condition of acquiring them?
