Nixon was entitled to a state funeral, but his family declined it. They thought it would cause controversy and they didn't want to deal with it. They chose to skip the state funeral and have only a private ceremony.
Other than Nixon, no other presidents have died since LBJ. Carter, Ford, GB 41, GB 43, and Clinton are all still alive. I am sure that you know what happened to JFK and the others before him were all dead before 1973.

Dunno if it's automatic (I assume it is), but if you still have friends in political power, you're getting one. Suffice to say, the presidency and both houses of Congress weren't going to say "nope, not for this one" on Reagan.

Can anyone tell me why in the world they feel the need to warn people? Isn't this like telling your opponent which play you're going to call in Tecmo Bowl II?
Unless they're running the Bear's Cap Bozo play. Then were all in big trouble.

I am not sure but I think that the President is the one who has the authority to order a state funeral. I don't think there are any rules about who the prez can and cannot authorize one for, but I could be wrong.

Burger already answered 1 & 2, I believe. As to your third question, I read about it this morning. Apparently State Funerals are available for Presidents, President-Elects and Former Presidents. I think it is a matter of the person's/family's choice as to whether they take it. I seem to remember the article saying there were State Funerals for Lincoln, Kennedy, Garfield and McKinley, so apparently that option isn't one taken frequently. I'll try to find the article.
