I think they should all get kicked to the curb, pompous asses. But I do disagree that your examples would be protected under the statute -- I read the statute (and related case law, which is unfair because I did not send it to you for review prior to posting - I will get right on that) as saying that you can't use Dr., M.D. or 'doctor' OR any other titles that imply you practice medicine. In other words, the first three are presumed to imply that.
If I remember correctly, there was some sort of carve-out in one case for professors so long as they kept it in the classroom -- but I believe that those pompous shits should be the first ones to get the boot.
I am so glad that almost everyone is on holiday and is not here to see this series of posts. May the god of the FB forgive us for our transgressions.

Ahh, yes. Thank you, I had forgotten about the conflict with the convention that year after they postponed that week. Although I'm surprised the NFL didn't go back to the no off week after that. Plus, the games without the layoff seemed sharper and more competitive.
Maybe they also had some issue with NFLPA, too? If that's the case, that's too bad. I quite enjoyed having to endure only one week of hype instead of two.

This is, indeed, good practice. Very few things irritate my parents more than receiving an invitation addressed to Dr. and Mrs. Balt Parents. Dr. and Dr. is acceptable (but not prefered); Mr. and Mrs. escapes notice. Balt Mother finds Mr. and Dr. to be amusing, but acknowledges that this is hypocritical.

I am so disappointed that God used the phrase "cease and desist." I would have hoped He had his minions of the dark (i.e., us) speak such nonsense (really, cease and desist -- such unnecessary jargon, Lord?) while he was more of an eloquent chairman of the board.

I'm sure that there's some regional and practice-specific variation on this, but I put "Esq." after other lawyers' names in correspondence and pleadings, but I do not put it after my own. In my view, it's kind of like the use of "-san" (or "-sama" or "-sensei") in Japanese* -- you always use it to refer to others, but never use it about yourself.
That being said, I see many many many lawyers using it for themselves as well as others, potentially running afoul of the pro se litigants who periodically file suit against judges and lawyers under the "titles of nobility" clause of the Constitution.
* The benefits of a liberal arts education. Or maybe an example of what happens when one waits until the last minute to satisfy one's humanities requirement, such that "Modern Japanese Culture 1862 to present" was literally one's only choice.
