Um, in her world, we "knew" she had given birth and "knew" she was on maternity leave so it apparently should have happened automatically without her having to do anything.
Her story has also changed from "I sent in the forms -- why don't you have them?" (to the customer service rep) to "How can I be expected to know I need to do anything to add my kid to my health plan, and my brother is a lawyer and he said you have a legal obligation to send me the forms I need to fill out without my having to request them" (to the customer service rep later, and to me; she shut the fuck up when I pointed out that I am an employee benefits lawyer and that her brother doesn't know what the hell he is talking about).
I do think we should do a better job of highlighting the fact that you need to add new kids/new spouses QUICKLY not just whenever you get around to thinking about it, but jesus christ, people are just irresponsible. And it tends to be people who make a LOT of money who are the most irresponsible.
In a sign that I may not be totally heartless, I usually let the babies in late.

The Marketing people at one of the firms I used to grace with my presence paid some goddamned hippy consultant tens of thousands of dollars to come up with a Visual Identity Protocol (tm) -- everyone in the firm was supposed to use the same font for all documents (MS wingdings, IIRC*), all correspondence was supposed to be in block format, and letters were supposed to be signed something like:
- Yr. most obd't svt,
Marks & Lennon
[illegible scrawl here]
For the firm
Needless to say, that was money well spent. And then, when a new managing partner came in, they paid another consultant even more money to completely revamp everything, including the stationery. There's an elementary school that is still, 2 years later, using the old version 100% cotton fiber Crane's letterhead for scrap paper and art projects.
*Yes, that was a joke. I don't remember what it was, only that my then-secretary was a huge fan of Times Roman, and she routinely flouted The Protocol as her way of thumbing her nose at the Man.
eta: She also used to edit my posts on the FB for me, and would never let me misspell the word "scrawl."

My friendly neighborhood HR person, who was totally excited about the baby, stayed in touch prior to my going out on maternity leave and I think I filled out a form in advance with my signature and then she filled in the details when I emailed her the news (from the hospital, on my blackberry, high on morphine). Had she not done that and been so highly attuned to the fact that I was about to have a newborn, I am not sure I would have been aware of a 31-day deadline, and in I am surprised to learn that it is so short, esp. in light of the blur that is that time period.
Since you have to take newborns for at least a couple of ped visits in the first month, however, I would think one would be reminded of the issue of insurance coverage.

That is what happened to us.
The 1st visit is like, within the two weeks of birth. We also rec. a bill not for my portion but the baby's.
So when I called HR, i called my plan to check if baby was added, no. So I call HR again and so on. but the trigger within 30 days was the bill.

I understand your point, it gets damned annoying and pointless at times. I enjoy some of the interview/analysis segments -- I tend to get most annoyed at the call-in segments, because I am not really interested in hearing what another average Joe thinks.

Yeah, some of the HR people suck in this decentralized, high-turnover place. And it is a weird time. Hence the bending of the rules.
People do a lot better when they say "I fucked up" rather than saying "my lawyer brother says it was your responsibility to keep me from fucking up," though.
