I think it might have been invented on the board. Possibly re-invented. I have not encountered the use of MMF vs MFM anywhere else. But I lead a sheltered life.
Even guys who break the taboos and are willing to allow as how having another guy around might not be absolutely out of the question seem to have more of a problem with MMF than MFM. ETA in my experience of discussing this. Which is obviously a very, very small sample size. Tiny.

I've had a few experiences where circumstances led me to believe the F wanted the MM or M M, but I never followed through. It just didn't seem right. I don't count the time in high school when I was fucking a friend's gf while he was passed out in the back seat and then woke up. They just sort of messed around, and the act was well underway by the time he woke up.
