I went to law school with a guy we nicknamed Inspector Clouseau. Here is his picture.

Generally $500 for one vial, which is enough for an area like your forehead. But it sounds to me like you have a medical need for it, so insurance might cover it. If not, they would probably charge you $500 because they need to charge for the whole opened vial even though you might just get one tiny shot.
