In addition, as you said, an A list actor's name comes before the title in the opening credits. Usually the film is advertised as, so and so in.... An A list actor chooses which movies will be a good vehicle for his or her career.
An A list musician is a person that sells out stadiums consistently. If they are in a band, just as many people must know their name as the band's name - Mick Jagger, Bono etc.
For politicians it is just anyone most Americans know their name. But remember, most Americans don't know who Dick Cheney is.
I would also put as an A celebrity anyone the Paparazzi covers twenty four seven. So I would put Paris Hilton as an A List Celeberty. Also Jessica Simpson and Lindsay Lohan. They are not A list actors, but A list celebrities. Princess Diana was an A list Celebrity. So is Donal Trump.
An A list sports star is anyone who is known nationally and is usually recognized by more people than their sports team. Many people were not aware of the Chicago Bulls, but most people knew Michael Jordan. I think Tiger Woods is the first A list golfer.
I would put as B list anyone that is not A list but whose name is recognized by most people in the country and is currently working.
C list would be has beens who most people recognize (Jacklyn Smith), or people that are currently working but only one section of the population recognizes them. But in that section they are big. Soap Stars, Professionnal Wrestlers etc.
D list is anyone whose career is of and on and they and are mostly recognized by a certain section of the population.

Pope is definitely an A lister.
Gerald Ford is not.
I think Saddam Hussein would be, but I am to close to politics to have an objective opinion.
The place where this started was with booking guests for the tonight show. Then it was how much you got paid to show up at partys. Paris Hilton gets $100,000.
For the tonight Show an A lister goes on whenver they want to go on.
B listers they try and book as much as possible. But they chase the B listers.
C listers are used to fill space left by the B listers. So C listers are still called and asked to be on the show.
D listers are called when they need someone at the last minute. They are on standby. Kathy Griffin did this for years for talk shows etc.
This is also the order in which they appear on the Tonight show.
All peripheral jobs in the entertainement industry are list add ons. He is an B list Accountant (he works for B lists types). He is a C list attorney (he represents C types). He is a A list agent (Ovits) etc.

I think it is hard to place someone like William Shatner. Not A-list, but seems to transcend B, C and D-list status.

William shatner would be low D, EXCEPT that he started getting odd-ball exposure. Maybe first on Stern then Priceline- combined with the solid geek fan base the new odd- ball exposure put him back as a solid B.
But while you are still just odd-ball, or geek-cult, you're D-list.

I am a huge trekkie, but I think he was a classic C until he won an Emmy. Now he is a B list. Whenever a movie came out he was on the B list for a while.
First you have to remeber that it is the 14-30 crowd that determine the lists. I once dated a girl in LA who had never seen an original Trek. Second, once your stop your career or take time off your place drops fast. Even if your show or movie was a cult hit.

C list because I think most people recognize him. As long as 8 out out ten people on the street recognize you (and can name you) you are on the C list.
