This is my theory.* I think that there are a lot of people out there that "save up" for plastic surgery and end up with crappy $3000 boob jobs. These people think that they were good because they were (to them) expensive. Insurance didn't cover the boob jobs, and these people may be paying them off of their credit cards for years. But they don't realize that the good boob jobs are waaaaaay out of their price range, and this is a case of getting what you pay for.
*A theory I'll never test because I'm more likely to get b'n'b's surgery than implants.

It has to go under a muscle, which pushes it flat at the eedges, giving it a "mound" appearance. Implants outside the muscle look better, and have the teardrop effect, but are a medical mightmare and feel awful. Most good surgeons won't even do them anymore.
