My understanding is that the danger is more to the mother than to the child, infection, pain and bleeding being the biggest issues. If the mother is dead you have very little time and a probably brain damaged child. If the mother is dying, the biggest risk to the child is likely being accidently cut when the mother instinctively flinches due to the pain.

But CNN and some other web sources suggest it was not regularly done, even when the mother was dead or dying, until the 1300-1500s. Was there just no understanding of the human anatomy? Not enough sharp knives? Aversion to the idea of cutting someone opn? Did the Kansas woman just get lucky and it's not really that easy and the baby usually dies without the proper technique? What?

ETA: There is some ancient "no man born of woman" could kill something or another myth where the loophole turned out to be that the hero was born via cesarian.

Also, remember that medicine has been separated from childbirth until very recently. There were some barriers to the trade set up by the docs/barbers, who weren't too interested in women learning how easy it is to cut people open.
